Chronic Disease Prevention program: Operating entirely on grant funding, this program is entrepreneurial and is designed to respond to issues identified by the community. The Chronis Disease Prevention program supports local, regional and statewide health-promotion projects that include expanded access to clinical, behavioral and social support—specifically those in policy adoption and clinical systems change.

Behavioral Health Promotion program: This program is a growing portfolio of grant-funded programs that engage the community to support behavioral health at the population level. It includes projects that focus on mental health promotion and substance misuse prevention with a mission of addressing the interconnectedness of such issues.
